Quick Relief of Hip Pain Gets Patient Back to Doing What He Loves

I had been seeing Mrs. DH for sometime when she mentioned that her husband was really having some difficulty getting out of bed because of some acute right hip pain. She wondered if I could help him the way I have helped her. We made an appointment for that afternoon.

Mr. KH indicated that he had been having right hip pain after the combination of a long hike followed by reclining on a sofa for several hours.  There was no trauma on the hike, and his only other injury was a left bone fracture in his foot many years ago.

His goals included being able to take the first step in the morning and being able to rise from a chair without severe pain (8/10) in his right hip region.  The evaluation in walking and standing showed his left heel counter was tipped to the inside and the heel was worn down on the inside of his shoe. It was also noted that the upper body was located behind the center of gravity of his pelvis.

Initial findings included restricted trunk rotation to the left more than to the right, reduced left hip motion on the right.  His straight leg raise was 75 degrees on the right and 55 degrees on the left.

The goals of treatment were to restore:

  1. Normal alignment to the pelvis and spine
  2. Stretch out the right side of the body to become as flexible as the left side
  3. Strengthening of the left abdominals and right buttock region
  4. Functional range of motion of the hips and spine in all planes

After the first visit, which emphasized activities and exercises to align the spine and pelvis in a neutral position, he was able to rise out of bed and get up out of chairs without pain.  The pain-free period continued for two months, after which the morning pain started again for no reason.  The second assessment revealed a twist in the pelvis so his home exercise program was modified.  He has remained pain-free and is back to walking 1-1.5 miles 5-6 times per week in new shoes that are replaced when they are excessively worn.

Long History of Shoulder Pain Comes to an End After Six Visits to Pinnacle PT

Pinnacle patient “MP” has a long history of right shoulder pain with overhead motions that has worsened over the last two years, now that she has become less active working with her horses. With an absence of trauma in her history, no x-rays were ordered in the physical exam by her physician.  Remarkably, the patient has only one kidney and has been advised to limit her exposure to anti-inflammatory medications.

The evaluation showed  limited and painful overhead motion with sharp pain when she reached across her body.  There was mild to moderate weakness of the rotator cuff muscles.

My patient started her physical therapy with the following SMART goals:

  • to attain neutral pelvic position and muscular balance at every joint
  • to increase her ability to expand her rib cage on the right side as well as she could on the left side with inhalation
  • to increase left abdominal strength particularly when she is exhaling
  • to improve left sided diaphragmatic breathing so it is similar to the right side
  • to increase right side bending to match left side bending of the trunk
  • to improve quality and quantity of right shoulder range of motion in all planes

On her second visit,  MP felt a significant difference in how much pain she was feeling and felt good about her choice to take care of this problem that had plagued her for 2 years.

After six weeks, MP was completely pain free in her right shoulder region (0/10).   She performed her home exercise program and worked with the physical therapist 6 times.  She was especially grateful that these effective exercises she followed were able to be performed with simple and inexpensive tools like fit balls, balloons, and resistance bands.
